Pregnancy weeks explained: why 40 weeks starts before conception

The most confusing fact about pregnancy dating is that week 1 happens before anyone is pregnant. Here is why the convention exists and what it means for your dates.

Every pregnancy app, every clinic and every scan report counts in weeks of gestational age. That number is measured from the first day of your last menstrual period — a date that precedes conception by about two weeks. The result is a system where you are notionally “4 weeks pregnant” at roughly the moment a test first turns positive, and where the embryo is always about a fortnight younger than the week number suggests.

Two clocks, both correct

Gestational age counts from the first day of the last menstrual period (LMP). It is the standard used universally in obstetric care, in scan reports and in the scheduling of screening tests.

Fetal age, sometimes called conceptional or embryonic age, counts from fertilisation. On a textbook 28-day cycle it runs exactly two weeks behind gestational age.

The reason clinics use the older, apparently sillier clock is entirely practical: the LMP is a date most people can name, and conception almost never is. Fertilisation cannot be observed, sperm survive in the reproductive tract for up to five days, and the fertile window spans roughly six days — so even someone tracking ovulation closely can rarely pin conception to a single day. Standardising on a knowable date is worth the two-week offset.

So a pregnancy described as 12 weeks involves an embryo that has been developing for about 10. Both numbers are right. If you read a fetal-development source that dates milestones from conception — some embryology texts do — subtract two weeks from the clinical week number before comparing.

What actually happens in the first four weeks

Naegele's rule and the cycle adjustment

The standard estimate is Naegele's rule: add 280 days — 40 weeks — to the first day of the LMP. The traditional formulation is “add one year, subtract three months, add seven days”, which gives the same answer for most dates and is easier to do in your head.

The rule embeds an assumption: a 28-day cycle with ovulation on day 14. If your cycles run consistently longer or shorter, ovulation shifts and the estimate shifts with it. The correction is simply the difference from 28: a 32-day cycle adds four days to the due date, a 24-day cycle subtracts four. This matters more than people expect — cycle length varies substantially between individuals, and a 35-day cycle moves the estimate a full week.

What does not vary much is the luteal phase, the interval from ovulation to the next period, which is close to 14 days for almost everyone. Cycle-length variation is nearly all in the follicular phase, before ovulation. That is exactly why the adjustment works: a longer cycle means a later ovulation, not a longer pregnancy.

How good is the estimate?

An estimated due date is the midpoint of a distribution, not a deadline. Only about 4% of babies are born on the exact date predicted. Roughly half arrive within a week either side, and the great majority within two. A birth is considered term anywhere from 37 to 42 completed weeks — a five-week window.

LMP-based dating is a good starting point when cycles are regular and the date is remembered accurately. It degrades where cycles are irregular, where hormonal contraception was stopped in the preceding months, where there was bleeding in early pregnancy that was mistaken for a period, or where the LMP is simply a guess.

First-trimester ultrasound is substantially more precise. Crown-rump length measured between about 8 and 13 weeks dates a pregnancy to within roughly five to seven days, because early embryos grow at a remarkably consistent rate. Clinical guidelines generally direct that the due date be revised to the scan estimate when the two disagree by more than about five to seven days in the first trimester. Later scans are less useful for dating — by the third trimester, normal variation in size swamps the signal, and a big baby is not a late one.

Trimesters, and why the boundaries are fuzzy

The conventional division is weeks 1–13, weeks 14–27 and weeks 28–40. You will see slightly different boundaries in different sources — some put the second trimester at 13 weeks, some end it at 26 — because the divisions are a descriptive convenience rather than a physiological event. Nothing happens at midnight on the boundary.

The dates that do carry clinical weight are specific and worth knowing: the dating scan at around 8–14 weeks, combined screening at around 11–14 weeks, the anomaly scan at around 18–21 weeks, and the threshold of viability, which sits somewhere around 22–24 weeks and has moved earlier over recent decades with improvements in neonatal care. Exact scheduling varies by country and by health system.

Size comparisons and what they are worth

Week-by-week fruit comparisons — a lime at 11 weeks, an ear of corn at 24 — are a communication device, not a measurement. They are drawn from published average lengths and weights, and real babies vary widely around those averages at every stage. A comparison chart cannot tell you anything about your pregnancy, and a baby measuring ahead of or behind the chart is, in the overwhelming majority of cases, simply a baby.

The one number that does carry information is the trend across your own scans, which is what your care team is watching. That is a comparison a calculator cannot make.

Working backwards from a due date

If a scan has already given you a due date and you want a tool that expects an LMP, subtract 280 days and enter the result with a 28-day cycle.
